Documents Required to Make Canara HSBC OBC Term Insurance Claim

The claimant/nominee should submit the following documents along with the prescribed claim forms to Canara HSBC OBC for the speedy claim processing.

The nominee/claimant should submit these following forms as applicable. These forms are readily available for download on the website.

• Death Claim Form (Form C)
• School/College Certificate (Form S)
• Physician’s Statement (Form P)
• Employer’s Certificate (Form E)
• Treating Hospital’s Certificate (Form H)

B. Mandatory Documents

• Original Policy Document
• Attested copy of the death certificate
• Death Claim Form – Form C
• NEFT Mandate with a cancelled cheque or bank passbook
• Nominee’s KYC details

C. Additional Documents, In the Case of Hospital Death.

• Physician’s Statement – Form P
• Treating Hospital Certificate – Form H
• Employer (Form E) Or School/College certificate (Form S)
• Hospital treatment records

D. Documents Necessary in the Case of Accidental/Unnatural Death

• Copy of Police FIR, Panchanama, and Police Complaint
• Copy of post-mortem report, autopsy, and viscera report
• Copy of Final Police Investigation report/charge sheet
