About Liberty Janata Personal Accident Health Insurance
Janata Personal Accident Policy offered by Liberty General Insurance offers coverage to the insured in case of a misfortunate event of death caused due to an accident. The minimum and maximum eligibility criteria to be covered under this policy for adults is 18 years and 75 years and for children is 5 years and 18 years respectively. The good news is that there is no exit age for this policy. This means that the policy is renewable for lifetime. The policy can be purchased for 1 year and can be renewed thereof by paying the required premium on time without any lapse. The policy offers you worldwide protection. Also, the policy comes with a free look period of 15 days during which you can read the terms and conditions of the policy to understand it better.

What's Included
- Cover for injury due to an accident resulting in death of the insured within one calendar year
- Cover for injury that results in loss of both sights or total and irrecoverable loss of use of one or two hands or two feet or one hand and one foot, etc.
- Cover for injury resulting due to accident within one calendar year due to loss of sight of one eye, or loss of one hand or foot, for 50%
- Cover in case of injury that leads to disability due to employment
What's Not Included
- Any loss arising due to the insured person committing any breach of law with criminal intent
- Death or disability resulting because of child birth or from pregnancy excluding ectopic pregnancy is not excluded from the plan
- Any claim due to war, invasion, act of foreign enemy, mutiny, rebellion, revolution, etc is not covered under the plan
- Any pre-existing condition/disability or accidental injury is not covered

Following are the documents required in case of personal accident death claims:
- Duly filled and signed claim form
- FIR/MLC from police authorities
- Driving license of the victim in case of death or injury because of road traffic accident and the victim was driving the vehicle involved
- Death certificate issued by competent authorities
- Death summary from the concerned authorities at the hospital, if the hospital confirms the death
- Post mortem report, if it has been conducted
- Documentary proof of death caused due to accident
- Inquest/panchnama report
- Photographs of the deceased/victim
- Letter from HR that mentions the attendance closure to the incident if the employee for group policies
- Latest photograph along with the photo ID proof of the beneficiary or legal heirs in whose name the payment is required to be made
- Coroner's report
- Succession certificate/legal heir certificate

Following are the documents required in case of personal accident injury claims:
- Duly filled and signed claim form
- FIR/MLC from police authorities
- Driving License of the victim in case of injury because of a Road Traffic accident and the victim was driving the vehicle involved.
- Medical certificate from the attending medical practitioner for the injury that indicates the extent of disability
- Medical fitness certificate from the treating consultant that specifies duration of rest advised by a medical practitioner
- Hospital/nursing home medical records
- Radiological/X-ray report confirming diagnosis or fracture
- For group policies, leave certificate from HR (for salaried people) if employee
- Photographs of the Insured showing affected area
- Photo ID proof of the deceased or victim in the name of whom the payment has to be made
- NEFT mandate form filled by deceased or victim in the name of whom the payment has to be made
- Salary certificate or income proof for employee in case of group policies
- Address proof of the deceased or the victim in the name of whom the payment is to be done
